UNDERSTAND IN 1 MINUTE ⏲ The Ministry of Health released, this Wednesday (20), the new protocol for the use of chloroquine in Brazil. The drug is now recommended for patients in the early stages of coronavirus infection, and at lower dosages. The new protocol officializes the use of the drug in the treatment of coronavirus in the public network for patients with mild conditions. Before, Ministry Health only recommended the prescription of chloroquine for severe cases, in which the patient is hospitalized, and with daily doses of up to 900 mg. Now, the recommendation is for lower daily doses for outpatients, that is, those who are not in hospital. This is a precautionary measure in view of the risk of side effects of chloroquine, especially cardiac arrhythmia. In the case of adult patients with mild signs and symptoms of Covid-19, the advice is to give a combination of chloroquine (450 mg) or hydroxychloroquine sulfate (400 mg) with azithromycin (500 mg) in the first days of treatment . The same guidance applies to patients with moderate signs and symptoms. For those with signs of severity, the protocol includes hospitalization and use of hydroxychloroquine sulfate with azithromycin. In children, according to the guidance, doctors should prioritize the use of hydroxychloroquine "due to the risk of chloroquine toxicity". For patients who have heart, liver or kidney disease, hematoporphyria and mental illness, the protocol is for chloroquine to be used "with caution". Despite guiding the use of the drug, the ministry admits, in the document, that there are no studies that "prove the unequivocal benefit of these medications in the treatment of Covid-19". "Thus, the prescription is prescription is at the doctor in charge discretion,  of the doctor, and the well informed patient's professed willingness  is also necessary".

ENTENDA EM 1 MINUTO ⏲ O Ministério da Saúde divulgou, nesta quarta-feira (20), o novo protocolo para uso da cloroquina no Brasil. Agora, o medicamento passa a ser recomendado para pacientes nos estágios iniciais da infecção por coronavírus, e em dosagens mais baixas. O novo protocolo oficializa o uso do medicamento no tratamento do coronavírus na rede pública para pacientes com quadros leves. Antes, a Saúde apenas recomendava a prescrição da cloroquina para casos graves, em que o paciente está internado, e com doses diárias de até 900 mg. Agora, a recomendação é de doses diárias mais baixas para os pacientes ambulatoriais, ou seja, aqueles que não estão em internamento. Essa é uma medida de precaução diante do risco de efeitos colaterais da cloroquina, sobretudo arritmia cardíaca. No caso de pacientes adultos com sinais e sintomas leves da Covid-19, a orientação é para que seja ministrada a combinação de cloroquina (450 mg) ou sulfato de hidroxicloroquina (400 mg) com azitromicina (500 mg) já nos primeiros dias de tratamento. A mesma orientação vale para pacientes com sinais e sintomas moderados. Já para os que têm sinais de gravidade, o protocolo inclui internação e uso do sulfato de hidroxicloroquina com azitromicina. Em crianças, de acordo com a orientação, os médicos devem priorizar o uso de hidroxicloroquina "pelo risco de toxicidade da cloroquina". Já para pacientes que têm doenças cardíacas, hepáticas ou renais, hematoporfiria e doenças mentais o protocolo é para que a cloroquina seja utilizada "com precaução". Apesar de orientar o uso do medicamento, o ministério admite, no documento, que não há estudos que "comprovem o benefício inequívoco dessas medicações no tratamento da Covid-19".
